Tina Tran, PharmD, assistant professor of Pharmacy Practice in the Department of Pharmacy Practice at Temple University School of Pharmacy, presenting “Community-based strategies to accelerate chronic disease management.”
Public health crises, such as the COVID-19 pandemic, lead to inevitable disruptions to essential healthcare delivery for chronic communicable (i.e., HIV) and non-communicable (i.e., hypertension, diabetes) diseases. However, one unexpected consequence of the pandemic may have been the acceleration of community-based models to ensure patient-centered care and timely delivery of treatment for chronic disease management. This session will discuss challenges and opportunities to expand community-based strategies to extend chronic disease care to hard-to-reach patient populations who face socioeconomic barriers.
